Come mezzi del potere, mafia e medicina condividono anche gli scambi ad hoc tra monocausalità e cause multiple. In medicina è prassi considerare una monocausalità quando le cause sono multiple. In modo da poter vendere trattamenti, giustificandoli come volti alla causa indicata come unica. Un esempio è la pressione in corso per gli screening neonatali*. Con la monocausalità si possono inoltre coprire cause multiple che si vogliono occultare, come i determinanti sociali della mortalità, a partire da povertà e deprivazione*. La monocausalità medica viene usata anche per mascherare manovre di potere inconfessabili**.
Vige la monocausalità della mafia “pantocratrice”***. Declinata dalla dx nella versione pura, “solo mafia”, e in quella “mafia con un pochino di deviazioni” da sn e magistrati. Ma sulle stragi nel cambio di repubblica, crimini smisurati e perentori, non ci viene detto come sia sparita la classe di ipotesi più semplice: di un’unica causa principale, necessaria e proporzionata. Es. le solite forze sovranazionali. Con i soliti autoctoni serventi. Invece la si butta, dopo 30 anni, nel gioco della sommatoria di cause multiple, cercando “concause” senza causa essenziale.

Professors Heneghan and Jefferson explain us the “reductio at binary” fallacy. While medicine suffers from and excess of application of the probabilistic framework, with statistics identifying with the fabled “scientific method”, it shuns quantification of chance when it should, as in the interpretation of lab test.
Binary thinking, so close to false dilemma, may be guarded against also for the ethical and political analysis. “Plot or panic” ? May be a mixture of the two, and of other components. Prominent among these, “false conscience”, itself an ambiguous mixture, and an universal attitude in medicine.

Asked by Tom Laughren, director of the FDA’s Division of Psychiatry Products, why AstraZeneca’s own drug trials didn’t “pick up a difference in sudden cardiac death,” Ray replied that AstraZeneca’s methodology suffered from “Simpson’s paradox,” pooling data from all clinical trials into one large data set with no allowances for the differences in the trials.
